tj: a nice good dentist i got my first wisdom teeth pulled out when i was about 16 and that old fart of a dentist gave me real pain than when i was about 23 i got my other one pulled the dentist was amazing he did it in like 5 seconds did not feel anything untill afterwards.

Skeeter: A Prosthodontist can do it all and many have labs on the premesis that can do the crown for the implant too. With an oral surgeon you will then have to go to a dentist or specialist who can create the crown.
